How to Apply for NHIF Online and Offline: Replacement

The National Hospital Insurance Fund (NHIF) serves as Kenya’s primary health insurance provider, aiming to facilitate access to quality and affordable healthcare for all citizens, as mandated by the National Hospital Insurance Fund Act.

Membership in NHIF is compulsory for those employed in the formal sector and voluntary for those in the informal sector or retirees.

Offline Application:

  1. Visit NHIF’s official website at http://www.nhif.or.ke
  2. Choose your category: employed or self-employed.
  3. Complete the employee application form with accurate information.
  4. Attach all necessary documents, preferably certified copies.
  5. Submit the form and attachments to the NHIF office.
  6. Once approved, collect your NHIF card from the office where you submitted your documents.

Online Application:

  1. Visit NHIF’s official website at http://www.nhif.or.ke/.
  2. Select your category: employed or self-employed.
  3. Fill out the online application form.
  4. Attach scanned copies of your ID card, passport photo, and marriage certificate, if applicable.
  5. Save and submit the form.
  6. You’ll receive an SMS regarding the status of your application.
  7. Upon approval, collect your NHIF card from any NHIF office or Huduma center.

Required Documents:

a) For Kenyan citizens:

  • National ID copies (including spouse, if applicable).
  • Recent passport-size photos (including spouses and dependents, if applicable).
  • The original and copies of birth certificates are required for dependents.
  • Employer appointment or introduction letter (excluding self-employed or voluntary contributors).

b) For Foreign Residents:

  • Passport copies.
  • The alien certificate or work permit (including spouse if applicable) is required.
  • Passport photos (including spouses and dependents, if applicable).

c) For foreign students:

  • A valid passport copy.
  • Passport-size photo.

Replacing a Lost NHIF Card:

If your NHIF card is lost, pay Ksh 100 at designated banks (National Bank, Equity Bank, Co-op Bank, or KCB) and present the bank slip to any NHIF branch or Huduma center for a replacement.

Adding a Spouse or Child to an NHIF Card:

To add your spouse, provide a copy of your ID card, marriage certificate, or affidavit at an NHIF or Huduma center. Present the birth certificate or notification for children aged 0 to 6 months.

Reactivating the NHIF Card for the Self-employed:

If your self-employed NHIF card is inactive due to non-contribution for twelve months or more, pay at least Ksh 1,500 to reactivate it. Resume card usage after two months. Payments can be made through mobile banking or at banks.
